What is the Access & Functional Needs (AFN) List?

The AFN list tells emergency responders who might need extra help in a disaster, like people with health issues, disabilities, or trouble with English.

Who should be on the AFN list?

A disaster can be hard for everyone, but some people need extra help. If any of these describe you or someone you know, our program can help you plan ahead.

You might need extra help if you:

  • Have health problems or need medical care.
  • Have mobility issues, like using a walker or wheelchair,
  • Don't have a car, or need help getting around.
  • Have trouble hearing, talking, or understanding others.
  • Need support with daily tasks.
  • Need help paying for emergency supplies.

This program is for people like:

  • Older adults
  • People with disabilities
  • People who speak a different language
  • People without a car or a way to get around

If you or someone you know could use this help, please sign up. We want to make sure everyone in our community is safe.

What information is collected?

We collect the nature of disability (to assess what supports are needed before, during, or after evacuation) as well as two emergency contacts. To view the full form, click here or click the button below. [1]

Who receives this information?

The information collected here will not be available to the public. It will only be shared with emergency response agencies to improve their ability to serve.

Can I fill it out for someone else?

Yes! We encourage you to fill this out on someone else’s behalf with their permission (if they’re able to give consent). This applies to parents, grandchildren, caregivers, and concerned family or friends.

This is not a substitute for personal preparation. In a disaster, government and other agencies may not be able to meet your needs. Make a plan and prepare for your care and safety in an emergency. You know your needs best, so prepare ahead with your family or caregivers.
